Section 64J-1.011 - Involuntary Inactive Certification(1) An EMT or paramedic certificate that is not renewed at the end of the 2-year certification period shall automatically revert to an inactive status for a period of no more than two additional 2-year renewal cycles.(2) EMT certificates may be reactivated if the applicant submits the renewal certification fee required by section 401.34, F.S., a late renewal fee of $25 and:(a) If applying for reactivation of the certificate within the first additional 2-year renewal cycle, verification of having met the recertification requirements contained in paragraph 64J-1.008(2)(a), F.A.C., or(b) If applying for reactivation of the certificate within the second additional 2-year renewal cycle, the applicant must verify having met the recertification requirements contained in paragraph 64J-1.008(2)(a), F.A.C., including an additional 30 hours of the refresher course training described in that rule paragraph, and must pass the department approved certification examination before the end of the second additional 2-year renewal cycle.(3) Paramedic certificates may be reactivated if the applicant submits the renewal certification fee required by section 401.34, F.S., a late renewal fee of $25, and: (a) If applying for reactivation of the certificate within the first additional 2-year renewal cycle, verification of having met the recertification requirements contained in paragraph 64J-1.009(2)(a), F.A.C., or(b) If applying for reactivation of the certificate within the second additional 2-year renewal cycle, the applicant must verify having met the recertification requirements contained in paragraph 64J-1.009(2)(a), F.A.C., including an additional 30 hours of the refresher course training described in that rule paragraph, and must pass the department approved certification examination before the end of the second additional 2-year renewal cycle.(4) All recertification training requirements required for reactivation of a certificate under this rule section must be completed before the application is filed.(5) Application for reactivation of a certificate under this rule must be made on Form DH 5023-MQA, 06/2017, Request for EMT/Paramedic Reactivation of Expired Certification, incorporated by reference and available at www.flhealthsource.gov and http://www.flrules.org/Gateway/reference.asp?No=Ref-09955.(6) An application for reactivation received by the department more than four years after the expiration date of the certificate shall be denied.
