Section 64B5-15.019 - Renewal Fees for General Anesthesia, Pediatric Moderate Sedation, and Parenteral Moderate Sedation Permits(1) The biennial renewal fee for a General Anesthesia Permit shall be $200.00.(2) The biennial renewal fee for a Moderate Sedation Permit shall be $200.00.(3) The biennial renewal fee for a Pediatric Moderate Sedation Permit shall be $200.00.(4) Renewal fees are not refundable.Fla.
