Section 64B5-15.017 - Application Fees for General Anesthesia, Pediatric Moderate Sedation, and Parenteral Moderate Sedation Permits(1) The application fee for a General Anesthesia Permit shall be $100.00.(2) The application fee for a Moderate Sedation Permit shall be $100.00.(3) The application fee for a Pediatric Moderate Sedation Permit shall be $100.00.(4) Application fees are not refundable.Fla.
