(2) Licensure application:

(a) Director - $90.

(b) Supervisor - $70.

(c) Technologist - $50.

(d) Technician - $25.

(3) Clinical laboratory personnel training program - $200.

(4) Continuing education provider - $250.

(5) Public health laboratory scientist - $25.
