Section 64B24-3.002 - Fees(1) The application fee shall be $200.00.(2) The 4-month prelicensure course application fee shall be $100.00.(3) The examination fee shall be paid to the examination vendor and refunded if the applicant is ineligible to sit for the examination.(4) The endorsement fee shall be $250.00.(5) The initial licensure fee shall be $500.00, whether by examination or endorsement.(6) The temporary certificate fee shall be $50.00 in addition to the fee required for licensure.(7) The active biennial renewal fee shall be $500.00.(8) The delinquent fee shall be $75.00.(9) The reactivation fee shall be $500.00.(10) The duplicate license fee shall be $25.00.(11) The continuing education provider application fee shall be $250.00.(12) The change of status fee shall be $75.00.(13) The inactive status renewal fee shall be $500.00.(14) The retired status fee shall be $50.00.(15) The unlicensed activity fee shall be $5.00.Fla.
