Section 64B2-11.001 - [Effective 12/29/2024] Application for Licensure Examination
(1) Any person desiring to be licensed as a chiropractor shall apply to the Department of Health on board approved form DH-MQA 1147, (Rev 08/24), Application for Chiropractic Physician Initial Licensure, http://www.flrules.org/Gateway/reference.asp?No=Ref-17177, which is hereby incorporated by reference, and may be obtained from the Board of Chiropractic Medicine, 4052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257, or apply online http://floridaschiropracticmedicine.gov/licensing.
(2) The licensure examination approved by the Board for licensure are the examinations developed and administered by the National Board of Chiropractic Examiners (hereafter NBCE) which consist of the Parts I, II, and III of the written examination, the Physiotherapy examination and Part IV, the practical examination; and the NBCE Florida Laws and Rules examination which measures an applicant's knowledge of Chapters 456 and 460, F.S., and rule Division 64B2, F.A.C. A score of three hundred seventy-five (375) shall be necessary on each section of Parts I, II, III, and IV of the NBCE Examination and the NBCE Physiotherapy Examination. A score of seventy-five percent (75%) is required on the NBCE Florida Laws and Rules examination.
(3) In order that the Board may timely certify to the Department of Health those applicants eligible for licensure, all applications, fees, and supporting documentation including transcripts, NBCE examination results, fingerprint information, verification of licensure from other jurisdictions and, if applicable, certified copies of court records evidencing a criminal conviction, plea, or other disposition, must be on file with the Board.
(4) For the purposes of Section 460.406(1)(d), F.S., the following organizations are Board-approved to evaluate applicants' education equivalency:
(a) Josef Silny & Associates International Educational Consultants;
(b) Foundation for International Services, Inc.;
(c) Education Credential Evaluators;
(d) International Consultants of Delaware, Inc.;
(f) Center for Applied Research, Evaluation & Education, Inc.;
(g) Foreign Academic Credentials Services, Inc.;
(h) World Education Services, Inc.;
(i) World Education Services SE Branch; and
(j) Any current member of the National Association of Credential Evaluation Services
