An applicant for licensure by endorsement pursuant to section 456.0145(2), F.S., shall submit as part of his or her application DH-MQA-5101(7/2024), "Mobile Opportunity by Interstate Licensure Endorsement (MOBILE)", which is incorporated herein by reference and which may be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-17155, the Board office, or at http://floridaspsychology.gov/applications/psychology-licensure-app.pdf. An applicant for licensure by endorsement must also demonstrate compliance with the criteria set forth in section 456.01452(2), F.S., prior to the issuance of the license.
