Section 64B18-12.006 - Inactive Licenses: Fees for Application, Reactivation and Renewal; Delinquency Fee
(1) The application fee for inactive status shall be three hundred fifty dollars ($350.00) if the application and fee are received by the Department during the timeframe established by the Department as the timeframe for biennial renewal of licensure; otherwise a fee of three hundred and fifty dollars ($350.00) shall be imposed in addition to the application fee.
(2) The fee for reactivation of an inactive license shall be three hundred fifty dollars ($350.00) plus the fee for biennial renewal of an active license as established in Rule 64B18-12.004, F.A.C., plus a fee of three hundred and fifty dollars ($350.00) if the application for reactivation is received by the Department at any time other than the timeframe established by the Department as the timeframe for biennial renewal of licensure.
(3) The fee for biennial renewal of an inactive license shall be three hundred fifty dollars ($350.00) if the fee is received by the Department during the timeframe established by the Department as the time for such renewal; otherwise a delinquency fee of three hundred and fifty dollars ($350.00) shall be imposed in addition to the renewal fee.
