Section 64B15-22.004 - Mandatory Registration of Unlicensed Physicians

Registration as a resident, intern, or fellow shall be accomplished by completing the board approved application form, DH-MQA 1172 (Revised 5/22), Osteopathic Physician in Training Application for Initial or Renewal Registration pursuant to Section 459.021, F.S., which is hereby incorporated by reference, and may be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-14608, from the Board of Osteopathic Medicine, 4052 Bald Cypress Way, Bin #C06, Tallahassee, Florida 32399-3256, or from the website at: https://floridasosteopathicmedicine.gov/.
