Section 61G10-13.009 - Reinstatement of Void Licenses

To reactivate a license that became void for reasons other than illness or hardship, the applicant shall complete Form DBPR LA 5, incorporated by reference in Rule 61-35.017, F.A.C., and submit it to the Department at the address on the Form.
