Rule 29-2825 - MATCHING OF BIRTH AND DEATH CERTIFICATES
2825.1

For purposes of matching a birth and death certificate, the following information about the decedent shall be provided to the Registrar so that it may be compared to the information on the birth certificate:

(a) Name of decedent;
(b) Name of father and maiden name of mother (except that for a birth out of wedlock the father's name will not appear);
(c) Date of birth or age of decedent; and
(d) The place of birth of decedent.
2825.2

No match shall be made unless there is documented proof of the fact of death.

2825.3

To implement a match, the date of death, the place (jurisdiction) where death occurred, and the death certificate number may be posted on the birth certificate.

2825.4

The birth certificate shall be marked to indicate the fact of death.
