The child-placing agency shall obtain an affidavit which sets out the identity of the father from the birth mother when the identity of the birth father is not reasonably ascertainable and there is no information to dispute the affidavit.
The child-placing agency shall obtain an affidavit which sets out the identity of the father from the birth mother if a named birth father's whereabouts are unknown.
