My Life, My Way program participants may decide at any time to voluntarily terminate their participation in the My Life, My Way program.
A My Life, My Way program participant shall indicate the decision to voluntarily terminate participation in the program by completing and submitting any required voluntary termination forms to DDS. The participant's authorized representative and/or support broker shall assist the participant to complete the forms as necessary.
Upon receipt of the participant's voluntary termination forms, DDS shall inform the participant's Service Coordinator and support broker of the participant's decision to terminate program participation.
Service Coordinators shall be responsible for assisting participants to transition to agency-based IFS waiver services. Service Coordinators shall ensure that there is no break in service provision during the transition period and shall coordinate the approval by DDS of the request to initiate agency-based IFS Waiver services.
