Behavioral Health Transition Planning services are services for Medicaid beneficiaries stepping down from certain institutional treatment settings. Behavioral Health Transition Planning services identify needed treatment and support services that aid recovery and reduce the chances of avoidable hospital or residential treatment readmissions, and connect beneficiaries to these identified treatment and support services.
Effective January 1, 2022, Medicaid reimbursable Behavioral Health Transition Planning services shall include the following activities:
Behavioral Health Transition Planning services must be rendered to the individual within the period beginning thirty (30) calendar days prior to discharge and concluding thirty (30) calendar days after discharge.
Medicaid beneficiaries who meet the requirements set forth in Chapter 65 of Title 22-A of the DCMR shall be eligible to receive Behavioral Health Transition Planning services.
Behavioral Health Transition Planning services shall be delivered in accordance with the requirements set forth in Chapter 65 of Title 22-A of the DCMR.
Behavioral Health Transition Planning service providers shall be certified in accordance with the requirements set forth in Chapters 65 of Title 22-A of the DCMR.
