Rule 26-B235 - DIRECTORS AND OFFICERS
235.1

The issuer shall list the names and addresses of all directors and officers of the issuer and all persons chosen to become directors or officers. Indicate all positions and offices with the issuer held by each person named, and the principal occupations during the past 5 years of each officer and each person chosen to become an officer. The issuer shall state the amount and type of securities of the issuer held by each person named as of a specified date within 30 days of the filing of the registration statement, and the amount of the securities covered by the registration statement to which he has indicated his intention to subscribe.

The issuer shall state whether any person chosen to become a director or officer has not consented to act as such.
