Appendix - Release of Mental Health Information for Outpatient Mental Health Treatment Form was re-codified and placed in section 8898 of subtitle A of title 26 of the Distirct of Columbia Municipal Regulations.

Authority: The Commissioner of the Department of Insurance, Securities and Banking ("Commissioner"), pursuant to the authority set forth in section 8 of the Prompt Payment Act of 2002 (Act), effective July 23, 2002, D.C. Law 14-176, D.C. Official Code § 31 -3137.
