Appendix 43-1: Uniform Consultation Referral Form was re-codified and placed in section 4398 of subtitle A of title 26 of the District of Columbia Municipal Regulations.

Authority: The Commissioner of the Department of Insurance, Securities and Banking, pursuant to the authority set forth in § 4 of the Uniform Consultation Referral Form Act of 2002, effective April 13, 2002 (D.C. Law 14-97; D.C. Official Code § 31-3233) (Supp. 2002).
