Licensees shall provide notice to the Department of intent to operate a barber, cosmetology, or personal grooming facility at least thirty (30) calendar days before beginning operations.
Licensees shall provide notice to the Department of intent to shut down permanently. A facility license and certificate of occupancy shall be returned to the Department and the licensee shall be required to submit a new application for the issuance of a new license prior to reopening.
Licensees shall notify the Department at least thirty (30) calendar days in advance of intent to close temporarily.
