The Director may either issue a special restricted permit or license or may set forth the restriction(s) upon the usual permit or license form.
At the time of examination of an applicant for a permit or license, visual acuity and hearing deficiencies, including the use of corrective lenses, shall be noted as a restriction on any permit or license issued.

Similarly, the necessity for special mechanical equipment or other permissible limiting conditions shall be noted on the permit or license.
Restrictions or limitations on the driving privileges of a person whose permit or license has been suspended or revoked shall be noted on that person's permit or license form.
