A party seeking to provide local exchange service in the District of Columbia shall file with the Office of the Commission Secretary an Application, which shall include a detailed Statement of Business Operations ("Statement"). A copy of the Application shall also be filed with the Office of the People's Counsel on the same date that the Application is filed.
The Application shall be accompanied by a check in the amount of $1,000 made payable to the Treasurer of the District of Columbia. This fee is non-refundable.
