Rule 10-B5802 - APPLICATION CONTENT
5802.1

In order to qualify for security deposit assistance, a Qualified High Technology Company must submit an application on forms prescribed by the Mayor.

5802.2

The application must be accompanied by the following information about the applicant and the proposed lease transaction:

(a) An original notarized affidavit, on a form prescribed by the Office of Tax and Revenue, certifying in good faith that the applicant is a Qualified High Technology Company;
(b) A copy of the organizing documents of the Qualified High Technology Company;
(c) A certificate of good standing to operate in the District of Columbia;
(d) The proposed lease and all addenda;
(e) A current balance sheet for the applicant;
(f) A cash flow projection for the applicant covering the current and next three fiscal years of operation;
(g) An income statement for the applicant;
(h) A proposed statement of commitments that will be undertaken by the applicant, in good faith, in exchange for any assistance granted; and
(i) Such other information as the Mayor deems appropriate.
5802.3

The applicant shall submit 3 copies of the application and attachments.

5802.4

An application fee in an amount to be determined by the Mayor shall accompany the application.
