Section 7301-11.0 - Well Identification Tag

11.1 Upon completion of the well and before leaving the site, the well driller, well driver, or pump installer shall fasten the well identification tag. The tag shall be permanently fastened to the well casing above finished grade by means of a one half or three-eighths inch stainless steel band or other device or method approved by the Department. The tag for a flush mount installation shall be permanently fastened to the top of the vault lid. The tag for an irrigation well with a concrete pad may be fastened to the pad.

11.2 The tag for a well permit issued by fax or under emergency circumstances shall be fastened to the well casing within five working days of the well driller's receipt of the tag.

11.3 Well tags shall be returned to the Department within 30 days of cancellation, revocation, or expiration of an unused permit, or the sealing of a tagged well.
