Such petition, when filed by an employer or his representative, shall contain the information required by the form supplied by the board, including the following:
(a) the name and address of the petitioning employer;(b) the general nature of the business and the approximate total number of employees;(c) the approximate percentage of business or sales outside the state of Connecticut, and other facts concerning interstate commerce, if any;(d) the types, classifications or groups of employees in the bargaining unit or units claimed to be appropriate, and the number of employees employed in such bargaining unit or units;(e) the names and addresses of any individuals or labor organizations who claim to represent any of the employees in the alleged bargaining unit or units, and a brief description of any contract covering any employees in such unit or units;(f) an allegation that a question or controversy exists concerning representation of employees between two or more labor organizations.Conn.
