Section 21a-235-11 - Records of permit holders

Each person to whom a sterilization permit is issued shall keep an accurate record of the name and address of each person other than the holder of such permit for whom sterilization is done, together with the kind and amount of articles and/or materials sterilized and the date of such sterilization. All records shall be open at all times during business hours for examination by the commissioner or by inspectors of the department, and certified copies of such records shall be furnished to the department not later than the fifth of each month, covering the preceding month.
