Section 20-332-20a - Fees(a) License fees shall not be prorated regardless of the date of issuance of the license.(b) The fee for replacement of a missing license is $5.00.(c) A person applying for the replacement of a license card allegedly lost, stolen or destroyed shall submit to the Department a notarized statement containing all pertinent information pursuant to the loss of license and shall pay the appropriate fee.
