A licensed provider who desires to renew a license shall submit before the expiration date an unaltered, completed and signed application for renewal accompanied by an application fee on forms prescribed by the commissioner Information requested at renewal includes, but is not limited to:
In the event that an unaltered, completed and signed application for the renewal of a license to operate a family child care home has been submitted on or prior to the expiration date, but has not been acted upon by the commissioner before the expiration date, the license shall remain in effect until the commissioner makes a decision on such application. During this period, all laws and regulations, as amended, governing the operation of a family child care home shall remain in effect and be binding upon the provider.
