Section 13b-17-200a - Form of petition(a) Any petition submitted pursuant to section 4-176 of the Connecticut General Statutes shall be sent to the office of the agency by mail or delivered in person during normal business hours. Such petition shall state with specificity: (1) the exact legal name of the Petitioner and the last known address of the principal place of business of each person;(2) the name, title, address and telephone number of the attorney or other person to whom correspondence in regard to the petition is to be mailed;(3) the substance and nature of the request; identifying the regulation, final decision or order concerning which the inquiry is made and shall identify the particular aspect to which the inquiry is directed.(b) The request for declaratory ruling shall be accompanied by a statement of any supporting data, facts, and arguments that support the position of the person making the inquiry.Conn.
