The record before the presiding officer shall include:
(1) All motions, applications, petitions, pleadings, notices of hearing and intermediate rulings;(2) The evidence received by the presiding officer;(3) Questions and offers of proof, objections and rulings thereon during the hearing;(4) The decision, opinion or report by the presiding officer.
