Section 2 CCR 405-2-222 - COLLISIONS, ACCIDENTS, AND CASUALTIES1. In the case of an accident involving a vessel, the operator thereof, shall file with the Division a full report concerning the accident. A vessel is considered to be involved in a boating accident whenever the occurrence results in damage, in excess of two thousand dollars, by or to the vessel or its equipment, in injury or loss of life to any person, or in the disappearance of any person from on board under circumstances which indicate the possibility of death or injury. Such boating accident includes, but is not limited to, capsizing, collision, foundering, flooding, fire, explosion, and the disappearance of a boat other than by theft. Accidents for the purpose of the report are only those which occur on the water.2. Reports required under subsection 1 of this section shall contain the following information:a. The number assigned to each vessel involved;b. The locality, time and date of the accident;c. The weather conditions existing at the time of the accident;d. The name, address, and age of each operator of a vessel involved in the accident;e. The name and address of the owner of each vessel involved in the accident;f. the name and address of any person who is injured or killed as the result of the accident;g. The nature and extent of injury to any person;h. A description of any property damage;i. A description of how the accident occurred;j. The type of vessel that is the subject of the report;k. The name and address, if known, of any witness to the accident.3.
