Section 12 CCR 2510-1-10.422.622 - SCSEP Termination Notices [Rev. eff. 12/1/10]

Termination notices shall include:

A. The effective date of the termination;

B. Reason(s) for the termination; and,

C. Appeal rights of the enrollee.
