Section RH 14.5 - Content of Application

In addition to the requirements set forth in RH 3.9 of these regulations, an application to receive from others, possess and dispose of wastes shall consist of general information, specific technical information, institutional information, and financial information as set forth in RH 14.6 through 14.10.

14.5.1 The licensee shall submit written statements under oath upon request of the Department, to enable the Department to determine the qualifications of individuals, the status of site operators, and whether or not additional actions at the site might be warranted.
