Section 170.215 - Service License and Vehicle Certification FeeA nonrefundable fee established by the Department, pursuant to M.G.L. c. 111C, § 3(b)(21), shall be submitted
(1) with the completed application for license form, and(2) upon acquisition of any additional vehicles during the licensure period. The fees are as follows:(A)Service License.(1) Ambulance BLS: $400.00 biennially(2) Ambulance ALS: $600.00 annually(3) Ambulance Critical Care Services: $750 annually(4) EFR service at EMS first response level only: $100.00 biennially(5) EFR service BLS: $150.00 biennially(6) EFR service ALS: $200.00 annually(B)Ambulance Vehicle and EFR Service Equipment Certification(1) Ambulance, BLS, ALS and critical care: $200.00 per vehicle, for each certificate of inspection.(2) EFR service, ALS: $50.00 for each certificate of inspection of each EFR vehicle's EMS equipment and supplies.Amended by Mass Register Issue 1486, eff. 1/6/2023.
