Section 164.515 - Change of Program Name(A) A Licensed or Approved Provider may not change the name of any program licensed pursuant to 105 CMR 164.000 without receiving express prior approval by the Department.(B) An application submitted by a Licensed or Approved Provider for a change in name must be filed with the Department at least 90 calendar days in advance in a manner prescribed by the Department.Adopted by Mass Register Issue 1482, eff. 11/11/2022.
