Section 125.012 - Required ReportingA licensee shall notify the Radiation Control Program of the Department within 30 days of:
(1) Any legal or disciplinary action or proceeding initiated against the licensee by any other licensing jurisdiction (United States or foreign), any health care institution, any professional society or association, including those listed in 105 CMR 125.005(C), any government agency, by any law enforcement agency, or any court for acts or conduct substantially the same as acts or conduct which would constitute grounds for suspension, denial, modification, limitation, revocation, or refusal to renew a license, or which would in any way relate directly or indirectly to his or her fitness to be licensed as a radiologic technologist; and(2) Any changes to the licensee's name or mailing address.Amended by Mass Register Issue 1333, eff. 2/24/2017.
