Section 1230.627 - Registration form and ballot(a)Producers.(1) A ballot (Form LS-72) and combined registration and certification form (Form LS-72-2) will be used for voting in-person. The information required on the registration form includes name, address, and telephone number. Form LS-72-2 also contains the certification statement referenced in § 1230.628 . The ballot will require producers to check a "yes" or "no."(2) A combined registration and voting form (Form LS-73) will be used for absentee voting.
