Section 1149.44 - What must documents filed with the ALJ include?(a) Documents filed with the ALJ must include:(b) Every document filed in the proceeding must contain: (1) A title, for example, "motion to quash subpoena";(2) A caption setting forth the title of the action; and(3) The case number assigned by the ALJ.(c) Every document must be signed by the filer, or his/her representative, and contain the address or telephone number of that person.
