Section 1261.305 - Form of claim(a) The official designated in § 1261.308 shall, prior to acting on a claim, require the claimant to submit a completed Standard Form 95-"Claim for Damage, Injury or Death."(b) NASA installations will furnish copies of Standard Form 95 upon request.
