The patient shall sign a release of information authorizing the hospital and the patient's providers, past and present, to release the patient's information related to the complaint to the Department. These records may include financial information, billing, medical, mental health, substance abuse, HIV, diagnostic imaging reports, and other records related to the complaint. A signed release of information is required for each complaint and will be valid until the investigation is closed or the release is revoked by the patient.
