Section 97305 - Voluntary Participation in the Program(a) To request to become a voluntarily participating entity, an entity or their authorized agent shall submit to the Department a written request to participate in the Program.(b) Each request shall provide the voluntarily participating entity's business type (as described in Section 97300(s)(1)-(6)), the number of covered lives, the types of coverage offered, and contact information.(c) The Department shall notify requestors if they are approved to participate in the Program.Cal.
