Section 75056 - Admission Records

(a) The clinic shall complete an admission record for each patient which shall include the following:

(1) Name.

(2) Current address.

(3) Age and date of birth.

(4) Sex.

(5) Date services began.

(6) Last date of services.

(7) Consent for treatment authorizations.
