Section 54205 - Physician Request(a) Adult day health care services shall be requested in writing by a physician. The request shall include:(1) Principal and significant diagnoses.(3) Specific type of treatment and anticipated duration.(4) Overall therapeutic goals for the participant.(5) Medications and special diets.Cal.
