Section 35277 - Documentation on the Child
(a) The agency shall obtain documentation on the child to facilitate the assessment, including:
(1) Indentifying information
(2) Medical reports
(3) Psychological evaluations
(4) Scholastic information
(5) Developmental history
(6) Family life history
(7) Certified copy of birth certificate
(8) Race and ethnic background
(9) Religion.
