(a) Supported Employment Programs offering group services may request permission to form new groups through submission to the Department of a completed Request for New SEP Groups, Form DS1962 (1/05),(b) The Department shall, in consultation with the vendoring regional center, approve or disapprove the formation of new groups based upon consideration of the following factors: (2) Ratio of consumers to job coaches;(3) Wages paid to consumers; and(4) Necessity for staggered hours attributable to medical and transportation needs as identified in the consumer's IPP, job requirements as established by the employer, availability of transportation in the geographic location.(c) Supported Employment Programs offering group services may request permission to change existing groups through submission to the Department and the vendoring regional center of a completed Notification of SEP Group Changes, Form DS 1963 (1/05). The following changes may be requested:(1) Increase or decrease in work hours or work days;(2) Implementation of or alteration to a staggered work schedule; and(3) Termination of group and transfer of consumers to another Supported Employment Program.(d) The Department and the regional center shall approve any proposed change to an existing program prior to implementation of the change by the Supported Employment Program.Cal.
