Section 58563 - Effective Date of Decision(a) The effective date shall be stated in the decision and shall be: (1) For the appeal of a denial of a request for amendment to a written contract, no earlier than the date the Department received the request for amendment;(2) For the appeal of a denial of a proposed rate based on a cost statement, no earlier than the date the Form DS2 (7/91), entitled Transportation Cost Statement, was received by the Department; or(3) For the appeal of a denial of a negotiated rate, no earlier than the date the information specified in 58540(c)(1) thru (3) was received by the Department.Cal.
