Section 13701 - Licenses for Frozen Food Locker Plants(a) Licenses for Frozen Food Locker Plants will be valid for a two-year period from date of issue and are not transferable. The fee for the license is $50.00.(b) Application for a Frozen Food Locker Plant license shall be made on State Department of Health form #EH-6.(c) The fee shall accompany the application and shall not be refundable.(d) The licensee shall immediately notify the Department of any change in the information which was submitted on the license application.Cal.
