Section 6504.4 - Specimen Collection Forms(a) Department approved specimen collection forms shall not be copied, printed, reproduced, acquired, purchased or distributed other than as provided for in this Group.(b) Such Department approved specimen collection forms shall be fully and accurately completed by birth attendants, perinatal licensed health facilities and laboratories; and a copy shall be filed in each newborn's medical record.Cal.
