Section 3104 - Filing of Names and Addresses(a) Each person holding a license from the Board shall file his or her proper and current mailing address with the Board at its office in Sacramento, and shall, within thirty (30) days notify the Board at its said office of any and all changes of name and mailing address, giving both his or her old and new name and address.(b) Each licensee shall also notify the board each time that he or she becomes the duly appointed administrator of a nursing home and each time that such an appointment terminates.
