Section 1043.2 - Composition of Onsite Inspection and Evaluation Teams
(a) An evaluation team shall consist of two or more persons chosen and approved by the Board for the first evaluation, or in the event that an applicant has failed an evaluation. For each subsequent evaluation, only one evaluator shall be required.
(b) The evaluators must meet one of the criteria in the Application for General Anesthesia Permit (Form GAP-1 New 05/2021) for general anesthesia or the criteria in Section 1647.3 of the Code for moderate sedation and must have utilized general anesthesia, deep sedation, or moderate sedation, whichever is applicable, in a dental practice setting for a minimum of three years immediately preceding their application to be an evaluator, exclusive of any general anesthesia, deep sedation, or moderate sedation training.
(c) At least one of the evaluators must have experience in evaluation of dentists administering general anesthesia, deep sedation, or moderate sedation. At least one member of the team must have substantial experience in the administration of the method of delivery of general anesthesia, deep sedation, or moderate sedation used by the dentist being evaluated.
(d) Evaluators shall possess a current, active, and unrestricted license from the Board or the Medical Board of California for applicants qualifying under Section 1646.9 of the Code. For purposes of this section, "unrestricted" means not subject to any disciplinary action such as revocation, suspension, or probation.
(e) The Board may appoint a licensee member of the Board to serve as a consultant at any evaluation.
