Reasonable grounds for cancellation of a certificate of self-insurance shall include, but not be limited to, the following:
(a) Failure of the holder of the certificate to maintain the requirements for obtaining a certificate as required by this article.(b) Inability, refusal or failure of the holder of the certificate to submit annual financial statements, the Financial Responsibility Self-Insurance Renewal Attachment, form SR70 (REV. 3/98) and requested supporting documentation as required by this article.(c) Submission of fraudulent or incomplete documents.Cal.
