Section 2699.6817 - Family Contribution Sponsor Registration(a) Family contribution sponsors must register with the Board.(b) To be registered, a family contribution sponsor must: (1) complete and return a Healthy Families Program Family Contribution Sponsor Registration form (HFP-Sponsor 1 (new 7/00)), which is hereby incorporated by this reference, and(2) receive a sponsor identification number from the Board.Cal.
